Please Note: If you are interested in fostering a specific dog to potentially adopt, please instead fill out the adoption application. We do a foster to adopt trial period for all adoptions. The foster application if for people who want to routinely foster and will be added to our foster emailing list and be contacted when we have foster needs.

Please fill out the form COMPLETELY and ACCURATELY! We cannot approve your application until we have all of the necessary information. Missing and inaccurate information causes delays with processing your application.

*Please note, we do background checks on ALL adults living in the home and need full names (including middle initial) and complete date of birth. Also, please call your vet and give permission for MUTT LIFE to access your records*

Lastly, if you are applying to foster a specific dog and eventually adopt if its a good fit, please fill out an adoption application instead! We do a foster-to-adopt trial period with all adoptions.
