Thank you for considering adopting a dog from us! Our adoption process is designed to ensure that dogs are placed in loving and responsible homes. Before applying to adopt, please review our adoption requirements carefully to ensure that you meet our criteria. We appreciate your interest in giving a dog a forever home!




ADOPTION PROCESS: Step by Step Guide

Review adoption requirements and ensure you meet the necessary criteria to apply!

Apply: Fill out adoption application completely and submit all necessary documents.

Application processing: we do a thorough background check, landlord approval (if applicable) or home ownership verification, vet check, and review your application. This process can take up to 5 business days.

note: if renting, please have your landlord write letter of approval for you to have a dog, including breed restrictions, and either send to muttlifeapps@gmail.com or submit document during application submission.​

Please authorize your vet to release medical information with Mutt Life. ​

Application Decision: approved applicants will be notified via phone call and email.

​Foster-to-Adopt Trial Period: after approval, you have the opportunity to do a meet and greet and bring your dog home! You get a 5 day foster-to-adopt (FTA) period to make sure its the right fit.

Final Decision: By the end of the FTA period, please let us know your final decision to adopt or return the dog to our rescue.

Home Visit: After the adoption process we conduct a home visit either in person or via zoom meeting to ensure that everything is going well with the transition and to answer any final questions you may have.

Neuter/spay Procedure: for dogs who are not fixed at the time of adoption, we will reimburse for the procedure once we receive proof of spay/neuter. The procedure MUST be done at HAWS Snip Clinic in Waukesha, WI or Wisconsin Community Veterinarian Center in Madison, WI. If you would like to go through your veterinarian or another location, we will reimburse $150 once we receive proof of the procedure. We recommend spay/neuter around 6 months old.

**Note: Adoptions are not considered final until the dog is neutered/spayed if applicable**
ADOPTION REQUIREMENTS

21 years of age or older

Reside in Wisconsin or surrounding areas (decided on a case-by-case basis for out of state)

All adults in the household must be in agreement to adopt the dog

Have a clear background check (including all adults in the household)

Demonstrate best-practice veterinary care on all current and previous pets (up to date on rabies, distemper/parvo vaccines, heart worm prevention, spay/neuter, etc.)​​

If renting, need written approval from your landlord to be considered for approval. (Approval should list any breed, weight, height, or pet-limit restrictions and sent to muttlifeapps@gmail.com or attach copy when submitting application)

Adhere to local zoning ordinances regarding pet limits.

If applying for a puppy under 6 months old, we require a plan to ensure they are not left home for more than 4 hours at a time.

we do not approve applications for dogs to reside outside. We believe that dogs are social animals that need to be part of a family and live indoors.
ADOPTION FEES
Adoption fees include the cost of spay/neuter procedure, up to date dewormer, vaccinations, heart worm prevention (age appropriate) and certified veterinarian inspection within 30 days of adoption.

$550: Dogs who are not spayed/neutered at the time of adoption. Spay/neuter cost is covered with the adoption fee and has to be done at HAWS Snip Clinic in Waukesha or Wisconsin Community Veterinary Center in Madison. Please send us the receipt for the procedure to get reimbursed. If you would like to go through your own vet, then we will reimburse $150 after proof of the procedure.

$500: all other puppies and dogs under 3 years old that are spayed/neutered before the time of adoption.

$400: Adult dogs: who are between 3-6 years old (who are spayed/neutered).

$300: Seniors: dogs who are older than 6 years old and already spayed/neutered
